Patient Condition Reports

St. Luke's policy is to provide one-word condition reports to media representatives. Requests must include the patient's full name. Requests will be denied if the patient or the family have requested that no information be released.

To receive a report during business hours (Monday–Friday, 8 a.m.–4:30 p.m.), contact Melissa Burlaga at 218.249.2988.

After business hours or for weekend condition reports, call 218.249.5555 and ask for the Administrative Supervisor.

Patient One-Word Condition Reports Definitions

Good: Vital signs are stable and within normal limits. Patient is conscious and comfortable. Indicators are favorable.

Fair: Vital signs are stable and within normal limits. Patient is conscious but may be uncomfortable. Indicators are favorable.

Serious: Vital signs may be unstable and not within normal limits. Patient is acutely ill. Indicators are questionable.

Critical: Vital signs are unstable and not within normal limits. Patient may be unconscious. Indicators are unfavorable.
