Aspirus St. Luke’s At Home is a Medicare-certified, Joint Commission accredited home health agency. We are licensed to provide home care services in Minnesota and Wisconsin, serving portions of St. Louis, Carlton and Lake counties in Minnesota, and portions of Douglas County in Wisconsin.

Our Team and Services

Whether you are coming home after a recent hospital stay, surgical procedure, or are unable to leave your home for medical care, the Aspirus St. Luke’s home care team offers a full range of home health services to care for you, including:

• Skilled Nursing: to provide health assessment, condition-specific education and training including diabetic care, cardiac care, catheter care and more
  • On-call Nursing: includes availability of nurse consultation 24 hours/day
  • Infusion therapy services: to manage intravenous medications, such as antibiotics or pain medications, per arrangement with your infusion company
  • Medication management: includes administration, assessment, and instruction (excludes ongoing medication set-up)
  • Pain management: care of acute and/or chronic pain including medication and non-medication treatment
  • Wound care: includes ostomy care and management
• Physical Therapy: to assess and provide treatment to help patients increase mobility, strength, balance and coordination
• Occupational Therapy: to help patients improve daily living skills such as eating, bathing, dressing and using adaptive devices
• Speech Therapy: to help patients restore or improve their communication skills including reading, writing and speaking
• Medical Social Services: to assist with social, financial, and emotional concerns, and coordinate community resources for support
• Home Health Aides: to assist with daily life activities and personal cares such as bathing and dressing, when provided with skilled nursing and/or therapy services

Aspirus St. Luke’s At Home does not provide psychiatric nursing, Personal Care Assistance (PCA) services, or non-skilled services only such as homemaking or home health aide only services.

Services offered by Aspirus St. Luke’s At Home are covered by a variety of sources including private pay, private insurance, Medicare, Medicaid, and other sources. If you have questions or concerns about coverage for your care, please contact your insurance provider directly, or contact us at 218.249.6111 and our staff will help you evaluate your payment options before receiving care.

These services are available to portions of St. Louis, Carlton and Lake counties in Minnesota. In Wisconsin, portions of Douglas county is served. Home Care is coordinated from this location.
